Golf Shaft Replacement Failed Twice !

A driver i bought at Dick's broke at the base of the shaft. The first repaired failed after 4 holes, the second repair failed after the first use. The club head keeps flying off. Not sure what Dick's is doing wrong when they attempt to repair the club...
